We have put together this packet in order to make surgery day as easy and stress-free as possible.  Please read and if you have the means to print and initial the last two pages to bring with you on surgery day that would be great.  If you do not have the means to print the documents, that’s fine we will have the paperwork ready for you to sign at admission.
